The Lectures Committee Support Request Form is designed using a series of html pages that allow an applicant to type the pertinent information on screen, then preview and print the page to obtain a hard copy. The form is not submitted electronically because applicants must provide nine sets of the completed application (the original plus eight copies of the Support Request Form, letters of co-sponsorship and evidence of the speaker's qualifications). The Support Request Form is also available in .pdf format.
